     - Salary pay stubs for the past 30 days
     - W-2s for the past two years
     - Verification of other income
    
If Self Employed
    
- 2 years corporate tax returns
     - 2 years personal tax returns (include all schedules and K-1's)
     - Signed & dated year-to-date profit & loss & balance sheet
ASSETS
     - Complete statements for last 2 months on all accounts
       (Savings, Checking, Money Market, IRA)
    
If Self Employed
    
- Complete statements for last 2 months for business accounts
CURRENT RESIDENCY & DEBT
    
- Name & phone number for current landlord or mortgage lender for the'
       past 2 years
     - Names, addresses and account numbers for all outstanding debts along
       with payment and balance due
     - If declared bankruptcy in last 10 years, copy of bankruptcy decree &
       discharge
     - If required to pay child support or alimony, a copy of the divorce decree
IF PURCHASING
    
- Check for appraisal and credit report in the amount of $405 payable to
       Mid America Mortgage
     - Copy of executed real estate sales contract
     - Name and phone number of your real estate agent
     - Name and phone number of insurance agent
IF REFINANCING
    
- Check for appraisal and credit report in amount of $405 payable to
       Mid America Mortgage
     - Copy of your most recent mortgage statement for your home loan
     - Copy of your homeowner's insurance policy declaration page
     - Name and phone number of your insurance agent
     - Copy of previous title policy
Signatures are REQUIRED on both the loan application
